Interoperability of Health Information Systems for Electronic Medical Record Data at Community Health Centers in North Banjarmasin District and the Banjarmasin City Health Office

Abstract

Health digital transformation in Indonesia prioritizes system integration to improve service quality. The level of interoperability of Electronic Medical Record (EMR) systems in primary healthcare facilities, particularly at community health centers (Puskesmas) in North Banjarmasin District, remains limited, resulting in data duplication and reporting inefficiencies. This study aims to analyze the interoperability conditions of EMR systems between Puskesmas in North Banjarmasin and the Banjarmasin City Health Office. The study employed a qualitative method with an exploratory case study approach. Data were collected through in-depth interviews with 19 informants (6 Heads of Puskesmas, 6 health information system managers, 6 IT staff, and 1 health information system official from the City Health Office) as well as field observations, and were subsequently analyzed thematically. Technical findings indicate that while basic infrastructure is adequate, system maintenance remains reactive, application integration (such as e-Puskesmas, P-Care, and SATUSEHAT) is still partial, and international interoperability standards (HL7-FHIR/SNOMED CT) have not been implemented. Non-technical findings show that the primary obstacles lie in the absence of standardized internal Standard Operating Procedures (SOPs), the lack of sustainable human resource training programs, and the absence of formal organizational structures to support cross-sectoral coordination.
